Joining The Practice
How to register
Joining the practice is easy. All eligible people living in the practice area can register. The practice area includes north Lewisham up to the river Thames ( New Cross, Deptford, Telegraph Hill, Brockley, Ladywell and some of Blackheath, St John’s and Greenwich). The receptionists can check the map to see if you live in the practice area. Even if you live just outside the area you may still be able to register. You need to fill in the application form which is available at reception and also complete a health questionnaire. If you provide your medical card that helps us to obtain your medical records more quickly but if you have lost your card you can still register. If you wish to change practices you can do so without having to give a reason. There will soon be a facility to partly complete the registration process on-line. Every patient who registers is offered a new patient health check with the practice nurse. Parents of children who are aged 5 or under should bring the child’s red book or immunisation information to the new patient check so that they can be given appropriate immunisation advice.
