Dear Patient

We have you coded as a person who is providing care to another person.

We would appreciate if you could complete the below form to help us ensure your records are up-to-date and to be able to provide you the right support and advice.

Failure to respond by 1st June 2022 means we will assume you are no longer a carer and will remove the carer code from your records.

If you would like to talk to someone about this, please call 020 3474 6730 and ask to speak to Lianne, the carers administrator.

Thank you
