Out of Area Patients

For Patients Who Have Been Informed That They Are Unsuitable To Stay Registered With Us

Thank you for notifying us of your change of address.

A doctor has reviewed your records and has stated that you are not suitable to stay registered as a patient at our surgery as you live beyond our catchment area.

At your initial change of address, you would have been sent a text and / or a letter to inform you that you are out of our catchment area which we cover, and guided you to review the terms of being registered as an out of area patient and what would happen next.

In considering a patient for out of area registration, it is very important that your individual health needs can be safely and effectively be provided by the GP practice, we have come to the professional opinion that it would be more clinically appropriate and practical for your specific health needs if you registered with a GP practice nearer to your home that can provide any home visits or urgent appointments more conveniently for you. 

 

What happens next?

Your details will be updated on our computer system and the Health Authority will remove you from our practice list (this will be within 30 days of receiving notification from the practice). 

Please note that all outstanding appointments after this date will be cancelled. 

We would advise that you register with another practice closer to home as soon as possible. 

If you need help in finding a local GP practice, you can access the NHS Choices website to find a new GP closer to your home address or telephone NHS 111 who can provide details of local NHS services for you.
